Improving Patient Safety In Rural Hospitals: A Workshop With Wisconsin Health Care Leaders (continued)
The increased rate of development of new drugs, and look-alike/sound-alike drug names has resulted in a dramatically increased rate of medication errors and adverse drug events if such products are prepared in the absence of a pharmacist. Patient morbidity and mortality have resulted from incorrectly prepared or contaminated sterile products. Recent ISMP publications and lay press articles (e.g. 9/00 Chicago Tribune nursing series) provide many specific examples in which sentinel events have occurred when nurses mistake one sterile product for another, as well as examples of patient infections resulting from poor aseptic technique. 4 ... The Next Influenza Pandemic: Lessons from Hong Kong, 1997
The current WHO global influenza program, with the help of four collaborating centers(Atlanta, London, Melbourne, and Tokyo) and 110 national influenza centers, aims tocentralize world data, study the epidemiology of the disease, and rapidly obtain newcirculating strains to make timely recommendations about the composition of the nextvaccine ( 31 ). However, many countries have only limitedcapabilities or resources to systematically search for and investigate unusual occurrencesof influenza. The events in 1997 in Hong Kong show the need to expand routine surveillanceefforts. Had the H5 virus isolated in May 1997 from a sporadic case not been identified inAugust, the reagents would not have been available locally to rapidly diagnose theadditional human cases of H5 influenza in humans in November and December. Without suchdiagnoses, and the investigations which they stimulated, authorities might not haveaddressed the issue of chicken influenza as they did. Transmission of the H5 virus to ... [Read More]
