One-to-One: Vermont's Nursing Home Transition Program Vermont used the federal grant monies under One-to-One to pay for home health agencies' initial expenses--such as training--when conducting nursing home assessments. The three coalitions that originally started One-to-One were also the first coalitions where home health agencies conducted nursing home assessments. DA&D gradually extended these duties to home health agencies in the rest of the state. The home health agencies used the same registered nurses who determine functional eligibility for the Medicaid HCBS waivers that are alternatives to nursing homes.
