Michigan FACE Report No. 03MI020 Earlier during the day of the incident he told one of the other custodians that he was going to get the “bulb gripper” extension pole to change the light. These bulb changes were often done with a man lift. The victim knew he was not certified to use the man lift, because his lack of training had prevented him from using the lift on other jobs. He had another conversation around noon with the school athletic director during which he indicated he was going to have to get an extension ladder, because he was not able to change the light with the extension pole. The light he had been trying to change was a floodlight type bulb. The extension pole with a “bulb gripper” did not work, because the gripper was designed for typical standard light bulbs, not the floodlight type bulbs in use. Michigan FACE Report No. 02MI016 On February 15, 2002, a 31-year old male rig hand/welder died from injuries sustained when he was pulled upward and entangled in the rotating drive shaft between the chain case and the rotary table in the substructure of the drilling rig. On February 15, 2002, MIFACE investigators were notified of the work-related fatality by the Michigan Occupational Safety and Health Act (MIOSHA) 24-hour-a-day fatality reporting hotline that a work-related death occurred on that day. The drilling company agreed to participate in the MIFACE program, and on March 8, 2002 a MIFACE researcher interviewed the owner of the company at the company headquarters. On March 21, 2002 the MIFACE researcher visited a company oilrig (not the rig involved in this incident) to interview personnel who were working with the victim on February 15, 2002. Michigan FACE Report No. 03MI079 On June 30, 2003, a 37-year-old male Department of Public Works (DPW) employee was electrocuted while attempting to read a water meter that was located behind an apartment boiler. The boiler room was lit by one overhead 40-watt light bulb. The meter was located near ground level. There was approximately a two-foot gap between the boiler and the back wall. He placed his water meter logbook on top of the boiler. Bracketed on a water pipe near the boiler was the boiler’s low water cutoff switch. It is unknown if the switch cover was on the switch when the victim tried to maneuver between the water pipes and the boiler to read the water meter. During his attempt to read the water meter, he contacted exposed, 120-volt energized low water cut-off switch terminals with his right chest. Michigan FACE Report No. 02MI07501 On May 30, 2002, a 30 year-old male died from hyperthermia (heat stroke) due to a multiple drug intoxication at the end of his work day. On July 5, 2002, MIFACE investigators were notified of the possible heat stroke fatality by the Occupational Health Division of Michigan Occupational Safety and Health Act (MIOSHA). On October 25, 2002, the MIFACE investigator interviewed the owner of the landscape company. The owner accompanied the investigator into the company’s garage area and showed her equipment typical of that the employees would have been using. With the backing of financiers, The Edison Electric Light Co. was formed on November 15 to carry out experiments with electric lights and to control any patents resulting from them.
