Institutional Risk (I) score is a numerical value from one (1) to five (5) representing the relative risk posed by the offender, to other offenders, Department of Corrections staff and State of Arizona property. This score is increased whenever and offender's behavior indicates a need for more frequent observation and more direct supervision of the offender's activities.  Offenders assessed Institutional Risk (I) scores of (I-1) will generally be placed in institutions where the least supervision and least frequent direct observation by staff is required.
